Postgraduate School in Gynecology and Obstetrics

Accredited for the period of 2002-2007 by the European Board and College of Obstetrics and Gynecology (EBCOG)


Statute

Art. 1.- (Institutions, Aims, Achievable Qualifications)

  • 1 - The Postgraduate School in Gynecology and Obstetrics has been established at the University of Studies of Siena in the following areas:
     o a) Gynecology and Obstetrics
     o b) Physiopathology of Human Reproduction
  • 2 - Its objective is to train specialists in the professional sector in the sciences of obstetrics and gynecology including the physiopathology of human reproduction. It follows the general norms of Postgraduate School in the area of Medicine.
  • 3 – The Postgraduate School grants the title of Specialist in Gynecology and Obstetrics
  • 4 – The University may also institute refresher courses subject to the conditions set by article 6 of Law 341/1990. For such courses the norms of the directive CEE 92/98, adopted by the legislative decree/vo n. 541/1992.

Art. 2.

  • 1- The Postgraduate Course in Gynecology and Obstetrics lasts five years, and its administrative office is at the University of Siena.
  • 2 – Each year of the course sets on average 200 hours of formal lessons and seminars and practical experience at medical facilities in teaching hospitals and/or agreed-upon hospitals until the set number of yearly full-time working hours in the national health care system (S.S.N.) is reached.
  • 3 – The operation of the Postgraduate School is regulated by the Faculty of Medicine and Surgery and, in particular, the Institute of Gynecology and Obstetrics as well as specific structures in the national health care system (S.S.N.) in the protocol of agreement art. 6 paragraph 2 of the Legislative Decree 502/92 and its relative university personnel belonging to the scientific-disciplinary sectors in table “A” and the managers of the S.S.N. of the corresponding functional areas and disciplines.
  • 4 - The agreed-upon hospital structures must adhere to all qualification requirements including art. 7 of the Legislative Decree n. 257/1991.
  • 5 – Hospital institutes and care facilities of a scientific nature functioning in coherent sectors with those of the postgraduate school must automatically meet these demands. The aforementioned non-university structures are identified with protocol of agreement art. 6, paragraph 2 of the Legislative Decree n. 502/1992.
  • 6 – Training must take place in university structures and in agreed-upon hospital structures to guarantee an appropriate theoretical preparation and suitable professional practical training, including work experience established by the EU norms (Law n. 428/1990 and Legislative Decree n. 257/1991).
  • 7 - Apart from the general criteria of the regulations of admission set by the current regulations and based on the human and financial resources and on the structures and equipment available, the Postgraduate School is able to accept a maximum number of four registrations for each year of the course for a total of 20 specialists. The actual number of registrations is determined by the national program, established in agreement with the Ministry of University and Scientific and Technological Research and the subsequent allotment of positions in the individual schools. The number of registrations at the school may not exceed the total number set by university educational regulations.
  • 8 – Graduates of the Degree program in Medicine and Surgery are admitted to participate in the admission process. Those who possess a title of study taken at a foreign university which is considered equivalent to Italian standards are also admitted. .

Art. 3. - (Plan of study and training)

  • 1 - The school board is obliged to determine the articulation of the postgraduate course and its relative plan of study in the different years and in the aforementioned structures.
    The school board, with the aim to fulfill art. 1 paragraph 2 and the objectives in paragraph 2 of this article and as specified in tables “A” and “B”, relative to the specific formative standards for postgraduate studies in Gynecology and Obstetrics, therefore determines, in respect to the rights of the patient:

        o a) the typology of the appropriate educational activities including practical laboratory work and work experience

        o b) the subdivision in periods of time theoretical educational activity, seminars, work experience and forms of tutorship

  • 2 – The plan of studies is determined by the board of the school with respect to general and specific subject area objectives as outlined in table "A". The organization of the process of training, including first-hand experience and the minimum necessary to obtain the diploma, is executed with respect to what is set for each individual specialization as specified in table "B".
  • 3 - The detailed plan of formative activity, previously in paragraphs 1 and 2, is determined by the board and made public in the annual manifesto of studies.

Art. 4. - (Annual program of activity and verification of training)

  • 1 - At the beginning of each year of the course the board plans the common and specific activities of the specialist students relative to training
  • 2 - For the entire length of postgraduate studies, specialist students are guided by tutors who are assigned annually by the board.
  • 3 - Training takes place in university structures and hospitals that conform to such agreements. Positive completion of training is certified by those who are responsible for training in the structures where such training takes place.
  • 4 - The board may authorize a period of time abroad in university structures or post-university structures consistent with the objectives of the school for a period of no more than one year.

 

Art. 5. - (Diploma Exam)

  • 1 - The final exam consists of a presentation of a written paper on a subject area assigned to the specializing student at least one year before the exam to be completed under the direction of one of the instructors of the school.
  • 2- The postgraduate diploma exam commission is established by the regulations of the school.
  • 3 - In order to be admitted to the final exam, the specializing student must have fulfilled attendance requirements in proportion to the set number of hours, must have passed yearly exams and training and must have progressively had first-hand experience in certified specialist medical activities according to the national standards outlined in table "B".

Art. 6. - (Agreement Protocol)

  • 1 - The University, as proposed by the Postgraduate School in Gynecology and Obstetrics and the Board of the Faculty of Medicine and Surgery, when dealing with more than one school for the same agreement, may establish agreement protocol according to the second paragraph of art. 6 of the Legislative Decree n. 502/1992, for the objectives described in art. 16 of the same legislative decree.
  • 2 - The University, as proposed by the Postgraduate School Board, may also establish agreements with public or private corporations with the objective to subsidize the activities and objectives of the school.

Art. 7. - (Definitive Norms)

Tables "A" and "B", which define the national standards for postgraduate school in Gynecology and Obstetrics (on the formative objectives and relative scientific-disciplinary sectors of pertinence and on the minimum activity of the specializing student for admission to the final exam), are decreed and updated by the Ministry of University and of Scientific and Technological Research, with the procedures of art. 9 of Law n. 341/1990.

AREAS OF PROFESSIONAL TRAINING AND RELATED SCIENTIFIC DISCIPLINARY SECTORS.

  • A. Preparatory Area
    • o Objective: the specializing student must learn fundamental knowledge of cellular and molecular biology of cellular differentiation and proliferation.
      Sectors: E04B Molecular Biology, E09B Histology, E11B Applied Biology, F03X Medical Genetics
  • B. Oncology
    • o Objective: the specializing student must acquire advanced knowledge of etiology-pathogenetic mechanisms which determine the development of the tumour.
      Sectors: F04A General Pathology, F04C Medical Oncology
  • C. Laboratory and Diagnostic Oncology
    • o Objective: the specializing student must acquire the fundamental theoretical and technical knowledge in the laboratory as it applies to obstetric and gynecological pathology including cytopathology, histopathology, and diagnostic imaging.
      Sectors: F04B Clinical Pathology, F06A Pathological Anatomy, F18X Diagnostic Imaging.
  • D. Medical Oncology
    • Objective: the specializing student must obtain a theoretical and technical knowledge of clinical practice necessary for epidemiological evaluation for the prevention, diagnosis, and treament of solid tumours.
      Sectors: F04B Clinical Pathology, F04C Medical Oncology
  • E. Epidemiology and Prevention
    • Objective: to learn the principals of epidemiology and oncologic preventative medicine.
      Sectors: F01X Medical Stastistics, F22A General and Applied Hygiene
  • F. Gynecology
    • Objective: the specializing student must acquire the necessary fundamental theoretical and technical knoweldge for diagnostics and therapy, in particular surgery of gynecological pathologies; and ultimately know how to participate in controlled clinical studies according to the norms of clinical practice.

  • G. Obstetrics
    • o Objective: the specializing student must obtain theoretical and practical knowledge as it applies to the physiology of pregnancy and birth, to diagnostic activities pertaining to maternal and fetal pathologies, to therapeutic activities, surgery, in particular, as indicated for such pathologies.

a) Gynecology and Obstetrics

  • H. Oncologic Gynecology
    • o Objective: the specializing student must obtain advanced theoretical knowledge and clinical practice necessary to diagnose and treat patients with tumours, even in critical stages.
      Sectors: F04C Medical Oncology, F18X Diagnostics through images and Radiotheraphy, F20X Gynecology and Obstetrics, F21X Anaesthesiology

b) Physiopathology of Human Reproduction

  • I. Physiopathology of Human Reproduction
    • o Objective: the specializing student must know how to put into practice the techniques of artifical fecundation with respect to the norms of the law and ethics.
      Sectors: E09E Histology, F07E Endocrinology, F20X Gynecology and Obstetrics, F22B Forensic Medicine

COMPREHENSIVE STANDARDS OF PROFESSIONAL TRAINING

In order to be admitted to the diploma final exam, the specializing student must demonstrate fulfillment of a complete and specific professional preparation based on having shown personal completion of specialist medical procedures as specified below:

  • Six months of general surgery;
  • Diagnostic and preventative activities in gynecological oncology in at least 250 cases;
  • Diagnostic and preventative activities in gravidic pathologies in at least 250 cases;
  • At least 50 major surgical procedures, of which 15% as primary surgeon;
  • At least 120 operations of medical surgery, of which at least 20% as primary surgeon;
  • At least 250 minor operations, of which at least 40% as primary surgeon;

For the specialization in Physiopathology of Human Reproduction, the surgery requirement is reduced to 20% and the specializing student must have completed at least 150 cases of articial fecundation, of which 25% as primary for the procedure.
Finally, the specializing student must have participated in the direction of at least three controlled clinical experiments according to the norms of clinical practice.
The typology of the different operations and their exact relative weight will be specified in the didactic regulations of each university.
